Ich bin über die Bedeutung einer eidesstattlichen Versicherung unterrichtet und belehrt, dass nach § 156 Strafgesetzbuch (StGB) mit Freiheitsstrafe bis zu drei Jahren oder mit Geldstrafe bestraft wird, wer vor einer zur Abnahme einer Versicherung Eides Statt zuständigen Behörde eine solche Versicherung falsch abgibt oder unter Berufung auf eine solche Versicherung falsch aussagt, und dass nach § 163 StGB mit Freiheitsstrafe bis zu einem Jahr oder Geldstrafe bestraft wird, wer eine solche Versicherung fahrlässig falsch abgibt. Mir ist bekannt, dass eine solche Versicherung auch vorliegt, wenn sie Angaben enthält, die den Tatsachen nicht entsprechen oder wenn Wesentliches verschwiegen wird.
Zur Erlangung der Betriebserlaubnis erkläre ich gegenüber dem Landratsamt Neustadt a.d.Aisch-Bad Windsheim an Eides Statt, dass ich keine Vereinbarungen getroffe habe, die gegen § 8 Satz 2, § 9 Abs. 1, § 10 oder § 11 Apothekengesetz (ApoG) verstoßen. Ich versichere an Eides Statt, dass dies nach bestem Wissen die reine Wahrheit ist und dass ich nichts verschwiegen habe.
